- Copy of Chapter Voter’s Registration – must be registered for six (6) months before applying
- Original Letter of Admission/Enrollment Verification
- Class Schedule w/Institution Heading
- Original High School/College Transcript
- Minimum of GPA of 2.0 – No Exceptions!
